Reablement care helps you regain independence following a change such as illness, injury or a fall.

Sometimes people need extra support to get back to their usual routines after a setback in their health. They may have been dependent on the help of others for some time, or lost confidence after a fall or injury. Our highly qualified carers offer an individualised service and compassionate support to help clients get back to living independently.

Reablement care at home

A reablement carer will support you to regain confidence and practical skills in carrying out daily activities. This could include:

Cooking healthy meals

Personal care, such as washing and using the toilet

Getting dressed

Mobility support

Specific medical help

At Greenwood Homecare, we understand that no two people are the same and will work with you to devise a care plan that fits your needs and preferences.

Who is reablement care for?

Reablement care is for anyone who has experienced a change in their circumstances or abilities, who might benefit from support to regain their independence. You may be looking for reablement care if you or your loved one:

Has recently been discharged from hospital

Is recovering from an illness or injury

Has experienced a change in living circumstances and support, for example due to bereavement

Professional support when you need it

Our friendly care team will discuss your situation with you to understand the level of support you need. We recognise that each person and each recovery is unique and that different services are necessary. As your requirements change, our flexible support can adapt to meet them.

Our aim is to help you gradually regain your independence. We will be guided by you and offer light touch support, ensuring you maintain autonomy as you recover.

Types of personal care

Our personal care plans are flexible to meet your needs, whether that’s a drop-in visit a few times a week or 24/7 live-in care. When discussing your care plan with you, we will take the time to understand your unique circumstances and the type of care that is right for you.

Visiting care involves your carer popping by for as little as half an hour. This type of care is helpful if you need assistance with specific activities or simply the peace of mind that someone is checking in. Longer visits can also be arranged if you require support throughout the day.

Night care ensures that you or a loved one can access the support you need overnight. A night carer can help with continence management, preventing falls or assisting with medication. They can also support with morning or nighttime routines.

If you or a loved one requires round-the-clock support, live-in care offers an alternative to care homes that puts the comfort and independence of the individual first. A live-in carer can be on hand whenever support is needed, providing assistance and companionship.

If your loved one is living with Dementia or Alzheimer’s disease, they may have complex care and emotional needs. Our highly trained carers are experienced in supporting people with the unique challenges of Dementia or Alzheimer’s disease, putting the safety, comfort and dignity of the individual first.

If you or a loved one is recovering from a fall, illness or operation, you may need reablement care while you get back on your feet. Our carers can help with medical needs and staying on top of things around the house. They also offer support and encouragement to build confidence and help you feel like yourself again.

Respite care allows carers to take a break, preventing carer burnout and offering variation to the person in their care. Respite care can be arranged for a day at a time or for several weeks.

Carers that care as much as you do

We know how difficult it can be to make decisions about care for a loved one. That’s why we are committed to hiring dedicated, empathetic carers who can make a real difference to the lives of the people they support. Our carers don’t simply provide assistance, but will support and empower your loved one to live as independently as possible.

From building your care plan, to each care visit, we take the time to understand your needs and ensure we are providing the very best care. Our friendly and dependable team are here to answer any questions you may have and are flexible to adapt your care plan as your needs change.

Reablement care is temporary, short-term care following an injury, operation or illness. It might involve helping you re-learn how to do your usual day-to-day activities such as cooking, washing or getting dressed.

At Greenwood Homecare, we will work with you to draw up a care plan that outlines your requirements and goals, including when and how often you would like support from a carer. Our reablement care services are flexible to adapt as you recover and we will regularly review your plan with you to ensure it continues to meet your requirements.

There are many benefits to choosing reablement care:

• Get help to regain your independence safely and effectively
• Receive compassionate support as you recover from illness or injury
• Prevent admission or readmission to hospital
• Recover in the comfort and familiar surroundings of your home

Reablement care typically lasts for a few weeks, but can be longer depending on your recovery. Our reablement carers will support you to gradually rebuild strength and independence and the length of your care plan is fully flexible to suit your needs.
